It might help for you to print these instructions. The credit application form is in pdf for download to your computer. Look in your Downloads folder for the filename. If Adobe Reader does not open the file when you double-click on its icon, you probably have an old version of the Reader. Download the newest version free at:

The application form can best be completed on your computer and printed to be signed. Or it can be printed for completion by hand in manuscript (printing). You may select each field (blank space) sequentially by placing your mouse cursor in the first field, then using the tab key to move to subsequent fields. Corrections can be made in earlier entries by placing the cursor in the entry field, deleting, and entering the data again. In the course table, you may skip the Prefix and the Number. Enter the full Course Title and the number of Units for which you are applying.

Notice that you probably cannot save the data to a file using Acrobat Reader.  Only the full version of the Acrobat program is designed to save data. The form might save, but you probably will lose what you entered.   Print the completed form for signing and sending after filling it in.

The signed form may be sent to AVLN by one of the following ways, in order of preference:

1. Via the postal service (From anywhere. Use airmail from outside the USA.).
